Current patients at Siloam should always come prepared for their appointments. Please read the information below to get ready for your next visit to our Primary Care Clinic.

What to Expect at Your Appointment

Are you ready for your first appointment? Here’s how to make sure!

Plan to arrive at least 30 minutes before your scheduled appointment time.

Gather any current medications you may be taking to bring them to the clinic for your appointment.

Pediatric patients must also bring a copy of your immunization records.

Make sure your Siloam paperwork (see forms below) is complete and with you when you arrive. This includes your Proof of Income form, health history questionnaire, and the patient information form. You can find all necessary patient forms here.

Adult patients are asked to pay $40 upon arriving for their first appointment. This covers the cost of seeing your medical provider and onsite social worker, as well as basic blood tests. New pediatric patients will be charged on a sliding fee scale depending on the type of visit and labwork needed.

After your first appointment, future appointment costs will be determined on a sliding scale based on the cost of services and household income level. Proof of income is required to be eligible for the sliding fee scale.
